Living Healthy/Tomando Control de Su Salud

The Health Foundation of South Florida funds the BRHPC’s implementation of the Living Healthy/Tomando Control de Su Salud, a program provided by the Healthy Aging Regional Collaborative. The health promotion and prevention programs selected for the Healthy Aging Regional Collaborative are designed to be implemented by lay leaders in community settings. These programs have been proven to be effective in helping older adults live healthier and more independent lives.  The Living Healthy/Tomando Control de Su Salud program is a model of the Stanford Self Management Programs. It consists of a six-week workshop, in English or Spanish, designed to empower seniors with various chronic diseases to take control of their health.  The program has been tested through both efficacy trials and effectiveness studies, which consistently demonstrate measurable improvements in self-rated health, social and role activities, symptom management, and communications with physicians, while also lowering health care costs. BRHPC works collaboratively with community partners to deliver the workshops throughout the Broward community.
